About the Role

Our client seeks a qualified Community Health Nurse to join their team, serving the Paarl area. This hybrid role involves providing essential health education, preventative care, and support services to individuals and families within the community. You will conduct health assessments, manage chronic conditions, and promote healthy lifestyles, balancing home visits and community outreach with remote administrative tasks and telehealth consultations.

• Conduct comprehensive health assessments, including vital signs, physical examinations, and health history.
• Provide health education on topics such as nutrition, hygiene, disease prevention, and chronic illness management.
• Administer immunizations and prescribed medications as per protocols.
• Monitor patients with chronic conditions, coordinating care and follow-up as needed.
• Facilitate health promotion activities and clinics within the community.
• Maintain accurate and confidential patient records, utilizing electronic health record systems.

• Registered Nurse (RN) with a current practicing license.
• Minimum of 3 years' experience in community health nursing, public health, or a related field.
• Strong clinical skills and knowledge of primary healthcare principles.
• Experience with telehealth platforms and remote patient monitoring is an advantage.
• Excellent communication, counseling, and cross-cultural competency skills.
• Valid driver's license and ability to travel within the Paarl community.
