Job Summary
Pharmacist – Lourdes Hospital is responsible for medication management activities that ensure safe, rational, and cost‑effective therapy while maintaining compliance with all applicable regulations. The pharmacist maintains departmental policies, ensures accurate medication preparation, oversees distribution functions, manages pharmacy inventory, and provides clinical support to patients and staff.
Essential Functions- Process orders and provide medication reviews for appropriateness, safety, and efficacy during order processing and perform any necessary clinical support duties.
- Ensure accurate preparation of compounded and non‑compounded products, handling them safely in accordance with regulations.
- Oversee distributive functions of the pharmacy department to facilitate accurate and timely medication availability to patients and staff.
- Manage pharmacy technicians and support personnel as required by the department.
- Maintain responsibility and accountability for the security of pharmacy inventory.
- Identify, document, and resolve potential and actual adverse medication events; evaluate reports in the event reporting system for trending and prevention.
- Provide medication education to improve patient understanding and adherence.
- Communicate verbally and in writing with pharmacy team members, healthcare providers, patients, caregivers, and leaders.
- Participate in departmental medication safety, quality assurance, drug utilization reviews, drug dosing, infection prevention, student precepting, and other activities as needed.
- Graduate of an ACPE accredited School of Pharmacy with a BS or Pharm
D (required). - Advanced pharmacy degree—Doctor of Pharmacy, Masters, Residency (preferred).
- Current license to practice pharmacy in the state(s) where working (required).
- Valid state‑issued (required).
- Immunization Certified (preferred).
- At least 2 years of recent experience in an acute care setting (required).
- Leadership experience of 1 year (preferred).
- Experience in pharmacy practice (required). One year of experience in pharmacy practice is preferred.
- Monitor patient conditions during treatment.
- Administer non‑intravenous medications.

- Core knowledge of medications, including adverse effects, interactions, proper dosing, monitoring parameters, and cost.

- EPIC Electronic Health Record (preferred).
- Proficient use of all computer hardware and software (required).
