About the Role

Longevity Health is looking for a Nurse Practitioner or Physician Assistant with a passion for working with seniors in long‑term care settings. In this role, you will be responsible for a panel of enrolled members receiving nursing home care and will be supported by a dedicated clinical leadership team that provides education and guidance on best practices for effectively caring for our members.

As part of your role, you will:

  • Provide collaborative primary care and services to a panel of Longevity Health members, treating them in the most appropriate setting based on the member’s goals of care.
  • Execute contracted plan’s model of care activities, including advanced care planning and implementing a treating‑in‑place model.
  • Facilitate and administer health plan clinical operations functions—prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promoting membership growth.
  • Serve as a trusted advisor to members, loved ones, and nursing home staff, educating care teams on member needs and early identification of changes in condition, notifying APP and telehealth partners for member changes.
  • Deliver effective and frequent communication and coordination with members’ PCPs, loved ones, and care team members on members’ care needs, goals of care, and treatment plans/changes.
  • Support overall market goals/targets and balance member caseload demands with other team member support needs.
  • Promote Longevity Health with members, loved ones, nursing facilities, and other stakeholders.
  • Function autonomously and be accountable for your practice.
  • Establish meaningful relationships and function effectively in the nursing home environment.
  • Be member‑centric and customer‑service oriented, ensuring high‑quality care for Longevity Health members.
  • Perform other related duties as assigned.
Credentials & Coverage
  • State‑unrestricted licensed Advanced Practice Nurse with national board certification (AANP, ANCC, or AACN) in Adult, Family, Geriatric, or Primary Care Nurse Practitioner. Alternatively, state‑unrestricted licensed Physician’s Assistant NCCPA board‑certified with nursing home/geriatric or hospice/palliative experience.
  • Active DEA license registered in the state of practice.
  • Active national provider number (NPI) and Medicare provider number, and eligibility to participate in the Medicare program.
  • Valid state driver’s license with a good driving record and proof of automobile insurance.
  • Will be willing to drive up to one hour to and from work daily and must be present in SNF communities/built‑up during business hours daily.
Education And Training
  • Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program or Master’s Degree from a nationally accredited program.
Knowledge And Experience
  • Minimum of 6 months experience as a Nurse Practitioner or Physician Assistant, with some experience in geriatric/hospice or palliative care populations prior to becoming an NP/PA.
  • Or a licensed and board‑certified new graduate NP/PA supervised by an experienced NP/PA in the LHP New Graduate Fellowship program for 6 months, successfully completing all competencies for independent practice as an LHP APP.
  • Experience working in a nursing home environment is strongly desired.
  • Computer/technology proficiency required.
  • Excellent people and communication skills, engaging effectively with physicians, patients, and family members.
  • Self‑directed yet collaborative within a team environment.
  • Excellent clinical skills—assessment, diagnosis, treatment, and prescribing, specifically for the chronically ill population.
  • Demonstrated critical thinking, organization, and prioritization skills to function independently.
