Nurse Practitioner

Nurse Practitioner General Medicine (SNF/ALF Setting)
Location: Paducah, KY and surrounding area (within 3540 minutes)
Schedule: Minimum 2 days/week (flexible schedule)

We are seeking an experienced Licensed Nurse Practitioner (NP) to provide internal medicine services in a Skilled Nursing Facility (SNF) and/or Assisted Living Facility (ALF) environment. This is an opportunity to step into a rewarding clinical leadership role as an on-site SNF‑ist, delivering consistent care to geriatric residents in long‑ and short‑term care settings. In this role, you will serve as the facility’s day‑to‑day provider, coordinating with nursing staff, medical directors, and attending physicians to oversee resident care.

Your work will directly impact patient outcomes, reduce unnecessary hospitalizations, and improve quality of life for aging populations.

• Provide direct patient care including health assessments, diagnosis, and treatment planning
• Perform monthly stability visits and acute condition evaluations
• Prescribe necessary medications and treatments within state scope of practice
• Communicate with facility staff, physicians, specialists, and family members
• Document all care provided using appropriate CPT codes and clinical documentation systems
• Participate in advanced care planning and patient care conferences
• Collaborate with interdisciplinary teams to deliver holistic care

• Active Advanced Practice Registered Nurse (APRN) license in the state of Kentucky
• Current DEA license
• Minimum 1 year of experience in a Skilled Nursing or Long‑Term Care setting (preferred)
• Excellent clinical assessment and documentation skills
• Strong communication and collaboration abilities
• Ability to work independently and manage a flexible schedule

Pay‑per‑encounter model Minimum commitment of 2 days per week (you choose the days) Opportunity for additional days as desired Single location in the Paducah, KY area
