Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona, including residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services for individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.
Responsibilities- Complete Medical Practitioner Admission Orders and Coordinate CON with the CPEC Medical Director within 24 hours of admission.
- Complete a psychiatric evaluation (HPI) and document within 24 hours of admission. Review and acknowledge completed physical health H&P of each patient.
- Review and modify treatment plans to meet psychiatric, medical detoxification, and physical health needs.
- Review cases with RN and Transition Coordinator daily during rounds as part of discharge planning.
- Perform daily rounds in inpatient setting between 08:00 and 16:00; document rounds in progress note.
- Monitor treatment of physical health and coordinate with PCP and other prescribers.
- Initiate or recommend consults and coordination of care.
- Coordinate patient discharge, admission, or transfer within the CBI system or to external facilities; document to ensure smooth transition.
- Be available to initiate coordination of care promptly and obtain collateral information as needed.
- Provide necessary documentation and treatment for patients under petition or court‑ordered treatment; coordinate with outpatient team.
- Discharge orders and plan: sign full discharge orders and follow‑up recommendations within 24 hours of discharge and review discharge plan with patient.
- Prescribe pharmacotherapy for psychiatric disorders and coordinate care for physical health, substance abuse treatment, and pain management in accordance with accepted standards of care.
- Maintain at least 80% e‑prescribing with a goal of 100%.
- Document addiction medicine, Suboxone taper, ongoing mental health/psychiatric care, and physical health evaluations prior to discharge.
- Complete orientation and shadow another CBI practitioner before patient care.
- Complete TB skin test prior to working with CBI patients; submit results annually.
- Obtain and maintain suboxone waiver for eligible patients.
- Maintain necessary patient data logs for buprenorphine treatment.
- Provide evidence of CPR/First Aid and renew every two years; submit renewal documentation.
- Participate in quality management initiatives and correct deficiencies within 72 hours; obtain approval as required.
- Maintain at least 85% compliance with documentation requirements.
- Do not cancel patient appointments without prior notification.
- Do not refuse to see CBI patients without approval from the medical director.
- Adhere to appointment scheduling protocols assigned by the medical director.
- Maintain minimum 85% patient satisfaction; submit plan of correction if performance falls below threshold.
- Maintain minimum 75% follow‑up rate.
- Complete and document all medication assessments and informed consents by end of shift or within 24 hours.
- Review Controlled Substance Prescription Monitoring Program prior to treatment and coordinate with other prescribers.
- Use e‑prescribing for all patient prescriptions as established in CBI’s electronic medical record.
- Attend regular staffing and rounds; participate in treatment plan development and provide clinical expertise.
- Support utilization review and management activities, documenting admission medical necessity and continued stay necessity.
- Support utilization management activities by providing reviews when requested by payors.
- Ensure documentation and treatment for patients under petition or court order.
- Promote collaboration with referring and receiving professionals.
- Attend practitioner meetings, state/regulatory/accreditation surveys, peer review activities, and performance improvement projects.
- Comply with agreed work schedules; provide coverage at facility to meet clinical and business needs.
- Give minimum 60 days’ notice for time off; coordinate coverage and submit requests as required.
