Overview
Job Summary: The post-holder will be a qualified nurse responsible for delivering practice nursing services to the practice population. Supported by the Lead Nurse within their scope of practice, they will demonstrate safe clinical decision-making and expert care for patients within the general practice. The role focuses on supporting patients to stay healthy, monitoring and managing long-term conditions, health prevention and screening activities, immunisations, and may include cervical screening.
The post-holder will work collaboratively with the multi-disciplinary general practice team to meet patient needs and support the delivery of policy and procedures. They will build effective working relationships with other health care professionals and work within a team of GPs, nurses and health care assistants to deliver excellent primary health care services.

Job responsibilities
- Primary Responsibilities:
Chronic disease management including diagnosis, monitoring and development of patient-specific management plans - Reviewing patients with chronic disease (e.g., diabetes, hypertension, heart disease) to implement and evaluate treatment plans to promote health and wellbeing
- Assessing lifestyle and medications and suggesting changes where appropriate
- Implementing processes for effective management of patients with long-term conditions using evidence-based practice
- Identify, manage and support patients at risk of developing long-term conditions
- Venepuncture, handling of specimens
- Undertaking diagnostic tests and screening procedures within practice protocols (e.g., ECG and cervical cytology)
- Immunisation and vaccination of children, adults and travellers
- Ear examination and syringing
- Wound care, dressings and removal of sutures
- First aid and management of emergencies (e.g., burns, haemorrhage, shock, cardiac arrest) and triage of injuries
- Care and maintenance of equipment
- New patient checks and ensuring appropriate signposting to the relevant clinician or external organisation
- Deliver opportunistic health promotion and understand practice policies for substance use and referral appropriately
- Maintain clear, concise documentation and ensure appropriate safety with all tasks
- Confidentiality in line with the practice policy and Data Protection Act 2018
- Maintain professional competency and be in date for NMC revalidation; ensure read codes are used effectively

Qualifications
- Registered nurse
- Membership of a professional body
- Evidence of CPD and training
- Evidence of continuous academic development
Experience
- Experience of working with clinical pathways
- Ability to work independently within boundaries
- Phlebotomy
- Experience as a practice nurse in primary care
- Knowledge and experience of long-term condition monitoring
- Wound care management
- Immunisations
- Cervical screening
- Knowledge of QOF
- Disclosure and Barring Service check required
- Current UK professional registration required
