Overview
The Intermediate Care Coordinator provides a consistent point of contact for members of the public and professionals, undertaking conversations to identify needs, risks and collating appropriate information for referral to the multi‑disciplinary team or other community services. The role includes carrying out social care assessments, applying the Care Act 2014 eligibility criteria and contributing to multi‑disciplinary assessments, while managing a short‑term caseload, formulating support plans and acting as a key worker.
Responsibilities- Provide a consistent point of contact and conduct conversations to identify needs and risks.
- Carry out social care assessments in line with the Care Act 2014 and other legislative requirements.
- Apply eligibility criteria and commission appropriate health and social care services.
- Collaborate with a multi‑disciplinary team to develop and update detailed support plans and set goals and objectives.
- Act as a key worker, monitoring needs and adjusting support plans as required.
- Coordinate a caseload and work alongside the multi‑disciplinary team, including coordinating hospital discharges and intermediate care caseloads.
- Research and recommend IT solutions to improve service delivery.
- Explain the financial assessment process to service users and refer to the Financial Assessments and Benefits team.
- Collect and collate information from a variety of sources, including individuals with communication difficulties.
- Analyse information to determine risk, priority and required actions.
- Direct incoming referrals to appropriate teams and ensure all mandatory training is kept up to date.
- NVQ Level 4 in Health and Social Care or an equivalent qualification (enrolment available within the role).
- Experience working in a health and social care setting with patients/service users and the public.
- Knowledge and understanding of the Care Act legislation and eligibility assessments.
- Excellent communication and information technology skills.
- Ability to multi‑task, prioritise and work autonomously in a demanding environment.
- Experience of working with carers, families and colleagues in a multi‑disciplinary setting.
This is a 37.5‑hour post within the Intermediate Care Team, working Monday to Friday between 8:00 am and 6:00 pm, with some weekend and bank holiday work required.
