ResponsibilitiesThis is an exciting opportunity for an enthusiastic and motivated advanced nurse practitioner to be pivotal to the continued implementation and development of Renfrewshire’s Hospital at Home Service. The candidate must demonstrate proven leadership skills and evidence of consolidation of advanced practice learning through continuous development across the 4 pillars of advanced practice. The candidate must be registered with NMC and have achieved an advanced practice qualification annotated on the SWISS register.
Significant experience as a practising ANP is advantageous for applying for this role. This role is clinical and will require a significant proportion of the lead ANP time to be clinical, as well as working alongside the service manager and key stakeholders related to operational pathway development and refinement to ensure robust clinical governance is in place.
The Renfrewshire hospital at home service is an integrated service with front‑door frailty teams within the RAH and is being developed in collaboration with East Renfrewshire HSCP. Patients admitted to the service are older adults living with frailty who present with an acute change to their baseline that requires acute‑level interventions that can be delivered service is preparing to expand to 18 beds delivered over 7 days, inclusive of public holidays.
The continuing development of the service will require the post holder to demonstrate strong leadership skills together with a high level of clinical experience to enable them to lead and develop the team accordingly.
