Staff Nurse Hospital

This is an exciting opportunity for enthusiastic and motivated registered community staff nurses to work within the developing Hospital at home service within Renfrewshire/East Renfrewshire HSCPs. Candidates must be registered on NMC prior to applying for this post.

The hospital at home team will work across the Renfrewshire HSCP geographical area and the Levern Valley area of East Renfrewshire. The hospital at home model is aligned to the frailty pathway with the team working closely with front door teams at the RAH inclusive of ED, MAU, AMU and the Older Adults assessment unit.

The post holders will be based at the RAH within the MDT hospital at home team working alongside acute teams and flow navigation plus colleagues to admit patients with frailty, presenting with acute changes, to hospital at home virtual ward where acute level care will be provided within the person's own home or homely environment.

The service over the next few months is preparing to expand to increase the bed model to 18 beds delivered over 7 days inclusive of public holidays.

The post holders will be integral to the management and care of patient admitted to the hospital at home virtual ward. They will work under the direction and supervision of the lead ANP and ANPs within the team and support the delivery of safe, high quality of care in the person's home. This post offers the right candidates the experience of delivering acute level interventions in people’s home or homely environment with the opportunity to develop further in the role as the service becomes more established.

• Shift pattern for this post are 8am to 8pm, 5 over 7 including public holidays

• The post holder is responsible and accountable for assessing, planning and implementation of programmes of care as directed by the ANP These will be developed in partnership with Patients, Family, Carers and Significant others, and will be evidence based taking into consideration the lifestyle, race, gender and cultural background, and factors which impact on physical, mental or emotional health.
• The post holder will be responsible for the utilisation of various clinical and environmental risk assessment tools in order to identify actual and potential risks and ensure implementation of appropriate interventions. The post holder will be responsible for reporting and participating in the critical appraisal of incidents to reduce any recurrences and participate in further investigation as required.
• As a member of the Hospital at Home team the post holder will be able to support patients, families and carers in order to achieve optimum health and independence.
• Plan and prioritise workload as required, delegate work using available resources and make decisions to ensure that patients’ needs are being met.
• Work within systems to ensure continuation of a quality Hospital at Home service
• The post holder may be required to provide care of patients within their own home or other homely setting eg. Care home
• The post holder will use a systematic, flexible and innovative approach to problem-solving where there is either barrier to understanding or financial constraints within the patient’s home, in order to provide a safe and effective care package.
• Participate in clinical audit and research that will ensure continuing improvements in practice and enhance individual and team performance for the improvement of patient care.
