NHS Lanarkshire is seeking an experienced Nurse, Midwife or Allied Health Professional (NMAHP) to lead frailty improvement across seven community hospitals. You will provide expert clinical advice, drive quality improvement and supervise the multidisciplinary team within a defined specialty.
The role involves partnership work with clinical teams, HSCPs, patients and third sector organisations to implement national Frailty Focus and Discharge without Delay initiatives.
