Job Function

An opportunity has arisen for a registered nurse to join the Health and Wellbeing Team. The post holder will support the delivery of high-quality care to young people with a range of needs, including complex health conditions, risk‑taking behaviours, neurodevelopmental disorders, and mental health difficulties. Working under the supervision of senior nurses, the post holder will contribute to health assessments, assist in identifying health needs, and support the implementation of health action plans in collaboration with child and youth care workers.

The role will include supporting the management of the electronic medication system and assisting in the organisation and delivery of onsite clinics.

Responsibilities
  • Support the safe management of medication, including the maintenance and monitoring of the Electronic Medication Management System.
  • Assist senior nurses in undertaking comprehensive health assessments for young people, including physical health, emotional wellbeing, medication, developmental needs and lifestyle factors.
  • Contribute to the development of health action plans and care planning documentation.
  • Support access to NHS outreach services within the safe centre setting (e.g. dental care, sexual health, immunisations, FCAMHS clinics) and encourage young people to engage with these services.
  • Contribute to the assessment of young people presenting with health concerns and escalate findings to senior nurses as appropriate.
  • Attend and contribute to multidisciplinary meetings and care reviews, sharing relevant health information.
  • Demonstrate an understanding of the determinants of health and support the delivery of health promotion activities and wellbeing initiatives in line with national guidance.
  • Support senior staff during critical incidents, including responding to self‑harm or suicidal behaviours, in line with agreed care plans and risk management strategies.
Communication and Relationships
  • Develop and maintain therapeutic relationships with young people in the residential setting.
  • Liaise with pharmacists and the GP practice team to coordinate the ordering and delivery of medication.
  • Communicate information regarding the management of mental and physical health needs clearly and appropriately to colleagues, multidisciplinary professionals, young people, and carers.
  • Maintain professional and effective communication with colleagues across the residential teams.
  • Provide support to young people who may be distressed, upset, angry or anxious.
  • Practice and promote confidentiality in line with the NMC Code of Conduct.
  • Ensure written documentation and records are clear, accurate and maintained in line with the NMC Code.
Qualifications, Skills and Experience
  • Essential:
    Registered Nurse with Nursing and Midwifery Council (NMC).
  • Desirable:
    Experience working with children or young people in a healthcare or residential setting.
  • Desirable:
    Experience using Electronic Medication Administration Systems.
Skills
  • Awareness of physical and mental health needs in young people.
  • Ability to recognise deterioration and elevate concerns appropriately.
  • Good organisational skills and attention to detail, particularly in relation to medication.
  • Confidence using electronic systems for documentation and medication management.
  • Understanding of safeguarding principles for children and young people.
  • Recognise own limitations and seek guidance from senior nurses when required.
