Job Description - Case Manager RN/LVN PRN Weekends/Holidays only ) )

Palestine Regional Medical Center

Works with Nursing Departments to perform daily review of assigned patient’s medical record for admission/treatment criteria, criteria for continued stay, and appropriateness and timeliness of treatment. Coordinates clinical reviews with third party payors. Queries attending physician for current plan of treatment, making suggestions when appropriate. Works with the primary physician and patient/caregiver to institute a discharge plan which will assist in meeting the patient’s physical, emotional, functional, and environmental needs.

• Minimum Experience - Three years clinical experience required.
• Ability to communicate effectively with patients, family members, physicians, third party payors and community resources is required.
• Problem solving skills required.
• Required Certification/Licensure - Must be currently licensed in the State of Texas to practice as a Registered Nurse or LVN.
• BLS certification required.
