Base pay range$/yr - $/yr
Major Responsibilities- Act as the Skilled Nursing Facility’s leading provider of internal medicine services.
- Collaborate with the facility’s interdisciplinary team and physicians, serving as a day‑to‑day clinical leader while overseeing the care of all residents.
- Work with short‑ and long‑term residents, managing their day‑to‑day care with nursing staff and guidance from medical directors and attending physicians.
- Evaluate new‑admission visits and coordinate discharge visits.
- Conduct monthly stability visits for long‑term residents.
- Perform evaluations for patients exhibiting changes in condition.
- Provide advanced care planning, comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol for acute and chronic illness.
- Prescribe all necessary medications and treatments.
- Perform comprehensive physical examinations.
- Develop and implement patient management policies and procedures.
- Facilitate referrals to other healthcare professionals and medical facilities.
- Communicate with attending physicians and/or specialists regarding patient care.

- Strong leadership experience with emphasis on teamwork in a healthcare setting.
- Master’s degree required.
