Knowledge of IMM, MOON, Advanced Directives, Multidisciplinary Rounds, Social Services, Value Based Programs, and other Care Management compliance guidelines [Required]
Ability to lead others effectively while maintaining appropriate compliance [Required]
Knowledge of Centers for Medicare & Medicaid Services (CMS) Conditions of Participation for discharge planning [Required]
Ability to manage multiple tasks and prioritize level of importance [Required]

Experience with PC applications and development of reports [Required]
