The Home Health RN is responsible for ensuring that The Key Concierge Nursing Services delivers a quality care experience to our clients and that client care plans are continuously updated through in‑person or virtual reassessments and reports. This role covers physical, emotional and social well‑being, providing life‑enriching opportunities and allowing as much independence as possible. The RN helps secure The Key as the provider of choice, supports smooth operations, client satisfaction, and long‑term retention of cases, working closely with colleagues in the immediate team and the larger community.
Responsibilities- Understand new clients, proactively identify areas of interest, reach out immediately after assignment, and schedule the first meeting to build a collaborative relationship with clients and families.
- Conduct in‑person initial assessments and subsequent reassessments, including home safety, daily routines, preferences, and needs, to create a care plan tailored to each client. Assess informal and formal support systems and initiate preventative and rehabilitative nursing procedures.
- Assist in coordinating all services provided.
- Provide services requiring nursing skills in accordance with the plan of treatment.
- Continuously ensure a positive client experience by building rapport and remaining highly visible to clients and their support systems.
- Make regular home visits, conduct reassessments as needed, update care plans when conditions change, and communicate updates to physicians or other qualified practitioners.
- Provide education and instruction to clients, families, or staff as required.
- Review client rates as care progresses.
- Ensure compliance with all state regulations, visit requirements, and documentation standards.
- Provide additional support for new client cases.
- Support ongoing client retention through hands‑on attention, appreciation initiatives, and relationship building.
- Apply home care knowledge, business goals, and care management goals that meet state home‑care regulations.
- Prepare documentation and clinical notes; submit notes per agency policy; respond to calls and messages per agency policy.
- Provide on‑call and after‑hours support as needed.
- Perform additional duties as assigned.
- Current Registered Nurse license in the state of employment; continued employment contingent upon maintaining an active license in good standing; BSN preferred.
- At least 2 years of experience in health care, elder care, social work, or a related industry.
- Excellent customer service and conflict resolution skills.
- Computer proficiency and ability to document timely and accurate notes in the system related to client visits.
- Current driver’s license and proof of insurance.
