Position Overview

- Coordinate resident care and medication management under the medical direction and supervision of attending physicians to ensure residents remain as independent as possible.
- Work on scheduled days off of the Health Services Director to provide consistent coverage seven days a week.
- Communicate with community healthcare professionals, including physicians, discharge planners, nurses, and state agencies on the care needs of all residents.
- Coordinate incidental medical, dental, vision, hearing, and podiatry care for residents.
- Perform nursing duties in accordance with the scope of the Nurse Practice Act, including administering medications and treatments as ordered by a physician.
- Support Medication Technicians to ensure competent delivery of medications to residents.
- Assist with resident assessment tasks as directed by the Health Services Director.
- Conduct staff interviews and observe residents continuously to inform staff of changes in residents’ physical and emotional health, and communicate relevant information to physicians and other healthcare professionals.
- Alert the Executive Director and responsible parties to concerns as needed.
- Complete thorough documentation in the resident chart on an ongoing basis according to company policy and state regulations.
- Review and monitor staff use of the Shift Report daily and participate in change‑of‑shift meetings to communicate pertinent information regarding resident care needs.
- Coordinate prescription orders with doctors’ offices and pharmacy delivery and oversee central storage and delivery of medications.
- Communicate with residents and responsible parties regarding the community’s preferred pharmacy and incontinence supply programs.
- Licensed Vocational or Registered Nurse holding a valid California nursing license in good standing.
- Prefer one year of experience working in a Residential Care Facility for the Elderly (RCFE).
- Proficient with a personal computer and MS Office (Word, Excel, etc.).
- Able to operate standard office equipment (fax machine, copier, telephone, etc.).
- Knowledge of or ability to learn the theory and practice of assisted living and dementia care.
- Comfortable working with seniors and patient interaction with cognitively impaired individuals.
- Maintain valid first aid and CPR certification.
- Pass Criminal Background Check and Health Screening tests, including physical and TB tests.
- Comply with Oakmont Management Group COVID‑19 policies, including vaccination, daily screening, surveillance testing, and PPE use per regulatory guidelines.
