Weekend Nurse Practitioner; Skilled Nursing

Major Responsibilties

• Provide internal medicine services on Saturday and Sunday as the SNF‑ist.
• Collaborate with interdisciplinary team and physicians to reduce weekend hospitalizations.
• Work with short‑ and long‑term residents alongside nursing staff and under medical directors/attending physicians.
• Evaluate new‑admission visits, coordinate discharge visits, and assess patients with changes in condition.
• Provide advanced care planning and comprehensive health assessments, differential diagnosis, and prescribe treatments under protocol.
• Prescribe all necessary medications and treatments.
• Perform comprehensive physical examinations of patients.
• Develop and implement patient management policies and procedures.
• Facilitate referrals to other healthcare professionals and medical facilities.
• Communicate with attending physicians and/or specialists regarding patient care.

• Advanced Practice Licensure (APN) with a Florida license.
• Must have an active license and DEA number.
• Previous experience in nursing or other medical fields.

• Excellent written and verbal communication skills.
• Strong leadership experience with emphasis on teamwork in a healthcare setting.
• Master’s degree required.
