Functions as the Education Coordinator. Coordinates the entire education program for the center and its affiliations. Oversees the development of appropriate educational materials, schedules, and plans outpatient programs (such as comprehensive education programs, staff development, support groups). Is responsible for coordinating application for ADA Recognition or AADE Accreditation and yearly updates. Chairs the Education Advisory Group and is responsible for coordinating the annual review process.
Provides diabetes in services and orientation to new staff, as well as other hospital staff. Functions as an educator – see below.
Assesses, plans, implements, and evaluates the patient centered diabetes education program at the hospital. Plans and conducts continuing education program in diabetes for professionals and the general public. Collaborates with other educators and providers in the areas of program development, implementation, marketing and review of clinical programs/services. Participates in the development of education curricula and materials by writing original material and/or reviewing the work of others.
Participates in case management, and patient care planning and goal setting. Provide diabetes education services and general diabetes self-management education (DSME) to patients and families, care partners and community.
Qualifications:
Graduate of an accredited school of nursing.
Current Certified Diabetes Educator
Demonstrated commitment to excellence in diabetes education.
Academic preparation and/or experiential preparation in program management.
Academic preparation and/or experiential preparation in the care of persons with chronic disease.
Proficiency with Microsoft Office or like software
