JOB DESCRIPTION:

One of the hospitals is looking for Emergency Medicine in Palmdale, CA

Emergency Department:

• 35 Bed Emergency Department + 8 Bed Fast Track
• 52 hours of physician coverage daily
• 56 hours of MLP coverage daily Back-up services:
  Cardiology, Inpatient Services, Surgery, Radiology
• MUST have a minimum of 2 years of ED Experience
• EMR:
  Cerner

Back-up services:
All subspecialties on site that you need except ENT, Ophthalmology, and OB but local support that is available for consult. STEMI-Receiving facility.

SUBMISSION REQUIREMENTS

• Provider must have a clean file and 7 plus shifts available per month.
• All candidates must have met the minimum shift availability listed on the job.
• CV must have all dates in MM/YY to MM/YY format
• Work History must be in Sequential order with most recent on top
• Any Gaps over 30 days must be explained on the CV
• EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help)
• All Licenses, Boards and Certs with expiration dates must be listed on CV
• Anyone with malpractice must provide a NPDB
• Anyone with legal issues must provide supporting documents

Availability

• 1st possible date they would be available?
• How many shifts would they be able for per month?
• What month is their schedule completely open?
• Any dates in the next 3-6 months they cant work?
• Any Holidays in the next 3-6 months they would be open to work.?
