Case Manager (WSP Shelter / Palmdale Dream Village)

Position Summary

Advancing Communities Together (ACT) operates the Palmdale Dream Village (PDV) and Winter Shelter Program (WSP) as part of its mission to break cycles of poverty, homelessness, and incarceration by providing safe housing and wraparound supportive services. The Case Manager is responsible for providing trauma-informed, client-centered case management services to individuals and families experiencing homelessness. This position focuses on helping participants stabilize, increase income, access mainstream benefits, and transition to permanent housing using a Housing First approach.

Program Director / Housing Coordinator

Essential Duties & Responsibilities
  • Provide intake, assessment, and individualized case management for assigned caseload in accordance with LAHSA standards.
  • Develop and monitor Individualized Service Plans (ISP) with clear housing and stability goals.
  • Connect participants to supportive services including:
    • Emergency/crisis intervention
    • Mental health and substance use treatment providers
    • Medical and dental care providers
    • Public benefits (Cal Fresh, GR, Medi-Cal, SSI, etc.)
    • Employment readiness, workforce training, and educational resources
  • Maintain accurate and timely documentation in HMIS/Clarity and internal databases.
  • Participate in Coordinated Entry System (CES) processes, including case conferencing and housing navigation.
  • Facilitate life skills and housing workshops (budgeting, tenant education, conflict resolution, leadership development).
  • Provide regular follow-up to track progress toward self-sufficiency and permanent housing placement.
  • Advocate for participants with landlords, property managers, and partner agencies.
  • Collaborate with program staff, LAHSA, and community partners to ensure seamless service delivery.
  • Maintain compliance with all HUD, LAHSA, and funder guidelines, including confidentiality, mandated reporting, and equity standards.
  • Participate in team meetings, trainings, and professional development required by ACT or funders.
Minimum Qualifications

Bachelor’s degree in Social Work, Human Services, Psychology, or related field OR 2+ years of relevant case management experience.

Knowledge of Housing First, Harm Reduction, and Trauma-Informed Care practices.

  • Experience working with unhoused individuals, transitional-aged youth (TAY), or justice-involved populations preferred.
  • Familiarity with HMIS/Clarity or similar case management systems is strongly desired.
  • Ability to de-escalate crises and respond with cultural humility and empathy.
  • Strong organizational, writing, and time-management skills.
  • Must have reliable transportation, a valid CA drivers license, and insurance.
  • Bilingual (Spanish/English) preferred.
Compensation & Schedule
  • Full-time, hourly: $22 – $24/hour depending on experience.
  • Schedule:

    Primarily Monday-Friday, with some evenings/weekends as needed for shelter operations.
