Physical Therapist (PT) Home Health – Full Time or Part Time

Join us at ABL Health Care as a Physical Therapist (PT) in our Home Health team in Palo Alto, CA. We are looking for a compassionate, organized, and team-oriented professional to provide exceptional care at patients’ homes.

As a PT you will evaluate patients’ physical health, develop individualized treatment plans, and work to improve mobility, strength, and overall quality of life. You will collaborate closely with our Clinical Director, Clinical Staff, and the Interdisciplinary Team, ensuring the highest standards of care in both remote and field settings.

• Provide PT services within the scope of care following individualized plans
• Administer treatments, monitor conditions, and report changes to physicians, case managers, or nursing director as needed
• Maintain accurate clinical records and submit documentation according to policy
• Comply with all company policies and procedures

• Current Physical Therapist (PT) License to practice in California
• Minimum 1 year of experience in Home Health or Hospice care
• Current California Driver’s License and ability to travel 15–25 miles regularly
