Based in Panama City, FL, serving patients in Bay County, F,L and surrounding areas.

• Travel to patients’ homes to provide personalized physical therapy services that support recovery and independence.
• Conduct thorough assessments to evaluate each patient’s condition and rehabilitation needs.
• Develop and implement goal-oriented treatment plans in collaboration with patients and caregivers.
• Guide and supervise Physical Therapy Assistants to ensure high-quality care.

• Unencumbered Florida Physical Therapy License (required).
• Valid driver’s license, auto insurance, and reliable transportation (required).
• Experience with OASIS documentation (preferred).
• CPR Certification (preferred).
• Ability to delegate and supervise Physical Therapy Assistants (preferred).
