Direct Support Professional – Sevita

Key Details
  • Shift: 6:30 a.m. – 2 p.m. and 2 p.m. – 10:30 p.m.; weekend availability required.
  • Rate: $15.00 per hour.
  • Must pass Level II background screening.
  • Full‑time or part‑time; non‑exempt, hourly role.
Summary
  • Position Type:
    Non‑exempt, full‑time or part‑time, hourly role.
  • Scope of Role:
    • Provide direct support within a single program or unit, typically supporting 4–6 individuals in residential settings, or up to 10 individuals in periodic programs.
  • Key Responsibilities:
    • Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
    • Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
Essential Job Functions & Services
  • Direct Support:
    Assist with socialization, behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living.
  • Records:
    Accurately document progress and activity; review records and logs; maintain confidentiality.
  • Management of Individuals’ Assets:
    Make cost‑effective purchases within budget and individual preferences.
  • Relationships:
    Maintain healthy and professional relationships with individuals, families, guardians, case managers; greet visitors; implement Sevita Customer Service Standards.
  • Reporting:
    Report incidents of alleged abuse, neglect, medical/behavioral events per Sevita policy and external requirements.
  • Rights:
    Respect confidentiality, rights of persons served, practice universal precautions, assist individuals in exercising their rights.
Health Care
  • Appointments:
    May accompany individuals to medical appointments; relay orders and information.
  • Medications:
    If assigned, accurately administer or support self‑administration; document delivery; report errors and maintain medication security.
  • General Health Care:
    Monitor health concerns; document; communicate with nurse or supervisor; call 911 in emergencies; follow care directives.
  • Medical Supplies & Equipment:
    Follow procedures, report malfunction.
  • Meal Preparation:
    Prepare or support meal preparation according to menus and individual choices; monitor mealtime.
Employment Responsibilities
  • Training:
    Attend orientation and ongoing training; participate in monthly staff meetings.
  • Employee Scheduling:
    Work scheduled hours; arrive on time; stay on shift until replacement arrives; notify supervisor when unable to work.
  • Teamwork:
    Work collaboratively, value others, be courteous, communicate openly, show respect.
  • Workplace Safety:
    Maintain safe workplace; comply with safety policies and procedures; report hazards.
Maintenance & Safety
  • Vehicles:
    Transport individuals safely; obey local laws and safety equipment; report accidents immediately.
  • Housekeeping:
    Perform daily & seasonal housekeeping; maintain clean, safe environment; report hazards.
  • Maintenance & Repair:
    Monitor environment; perform minor repairs; report equipment/facilities needing repair.
  • Safety:
    Check water temperature; participate in safety drills; protect individuals in emergencies.
Other
  • Perform other duties as required.
  • MPA = Mobile Punch Authorized for timekeeping.
Supervisory Responsibilities
  • None.
Education & Experience
  • High school diploma or equivalent (preferred).
  • Six months of experience in human services (preferred).
  • Must be 18 years of age.
  • Working knowledge of computers.
Certificates, Licenses, and Registrations
  • Valid driver’s license in good standing.
  • Car registration and insurance if providing transportation.
  • All state‑required training(s) and certification(s) completed timely.
  • Maintain valid driver’s license and insurance; approved as a company driver; compliance with MVR requirements.
Work Environment
  • Works at the program location and may accompany individuals into the community.
