Transitional Care Nurse

We are passionate about helping kids thrive at home and are looking for a Transitional Care Nurse who shares that passion to help transition medically-fragile children from the in-patient to home setting.

• Location: Omaha, NE
• Title: Transitional Care Nurse
• Schedule: Full Time
• Shift: Flexible to work avariety of days and shifts around the Omaha metro area, to support the unique needs of patients transitioning to the home setting. When possible, schedules will be created 2 weeks in advance. Differentials are available for night and weekend shifts.
• Position will support a patient and family as they transition to the home setting for the first time, after a hospitalization or new diagnosis/technology. The Transitional Care Nurse ensures the journey from hospital to home is a smooth process, starting in the hospital with bedside training to being settled at home and ready to hand off cares to the family and long‑term nursing team.
  
  The Transitional Care Nurse helps the patient and family be comfortable and confident at home after discharge.

• Must be a Registered Nurse licensed in good standing with Nebraska state board of nursing.
• Two years of experience working as a RN, pediatric experience required.
• ICU or other critical care level experience preferred.
• Experience training or providing education to nurses is preferred.
