Physical Therapist – Care One

Location:

Paramus, NJ

Facility:
Care One at Ridgewood Avenue

Salary range: $40.00 - $63.00 per hour

Practice setting:
Short Term Rehab, Long Term Care

Employment type:

Responsibilities
  • Provide and direct resident rehabilitation care using accepted practice methods, ensuring compliance with all center, state, federal, and accrediting standards.
  • Evaluate resident needs and develop effective, efficient treatment plans for assigned residents.
  • Coordinate and supervise treatment plans with interdisciplinary staff and other disciplines as required.
  • Participate in resident screening according to directed schedules and practices.
  • Provide clinical and rehabilitation feedback to the designated supervisor.
  • Participate in all center‑directed processes and services.
  • Perform necessary clinical documentation to meet center, state, federal, and accrediting agency requirements.
  • Maintain complete, accurate, and timely billing records to meet center, state, federal, and accrediting agency requirements.
  • Participate in in‑service training programs as requested.
  • May travel to other facilities to provide services on a limited basis.
  • Collaborate daily with nursing and clinical staff to strengthen patient outcomes.

Qualifications
  • Master’s or Doctorate Degree in Physical Therapy.
  • Current and active Physical Therapist license in the state of operations.
  • Prefer experience or knowledge in older adult rehabilitation.
  • Prefer knowledge of governmental and regulatory guidelines in designated healthcare settings.
