Job Duties
  • Provide social work services, including assessment, counseling, consultation, planning, community organization, policy, administration, research, and client-centered advocacy.
  • Deliver clinical social work services under the supervision of an LCSW, including clinical assessment, consultation, psychotherapeutic counseling, and client-centered advocacy.
  • Supervise students completing undergraduate or graduate coursework in accordance with Council on Social Work Education standards.
  • Provide counseling to clients individually and in groups around emotional and mental health issues.
  • Build and maintain strong rapport with clients in both individual and group settings.
  • Provide accurate clinical assessments, including mental, emotional, and general physical assessments for substance abuse cases.
  • Keep detailed, concise records of each client’s treatment.
  • Conduct professional consultations with medical professionals, therapists, physicians, and court officers as appropriate.
  • Follow directions for research and conduct screenings, intake assessments, and consultations for individual and family dynamics and treatment needs.
  • Provide ongoing social work intervention and education to patients, families, and significant others for health-related psychosocial problems aligned with team goals.
  • Utilize individual, family, group work, and crisis intervention methods to identify and address problems influencing treatment participation and discharge planning.
  • Complete comprehensive psychosocial histories promptly.
  • Offer direct treatment services using appropriate modalities, including individual, family, and group counseling, and crisis intervention.
  • Serve as a resource to the healthcare team on chemical dependence, psychiatric, and abuse issues; identify, assess, treat, and refer patients to community services for child abuse, domestic violence, sexual assault, and elder abuse.
  • Provide case management services for assigned caseloads to ensure treatment plans are followed and patients receive supportive services.
  • Interpret illness nature and treatment courses for patients and families, communicating family issues to staff.
  • Act as liaison to facilitate family dynamics understanding and timely discharges.
  • Advocate for patients within Medical Center programs and liaise with community providers to facilitate linkages.
  • Make presentations on field issues and Medical Center resources for community outreach.
  • Participate in interdisciplinary diagnostic and treatment planning conferences, recommending social work perspectives.
  • Fulfill case recording, statistical reporting, and other documentation requirements accurately.
  • Provide clinical supervision to paraprofessional staff and students as assigned.
  • Offer consultative services to Evergreen Outpatient Program, Medical Center staff, and community agencies on social work issues and services.
  • Assist in developing discharge care plans and suggest appropriate community resources.
  • Deliver assessment, treatment, and patient care with age-appropriate knowledge of development.
  • Maintain professional customer service, responding timely, professionally, caring, and respectfully per performance expectations.
  • Maintain departmental policies, quality assurance, safety, environmental, and infection control standards, identifying gaps and creating policies to address them.
  • Adhere to the Medical Center’s Code of Conduct and uphold its Mission, Vision, and Values Statements.
