Shift: Day Shift | 5x8s (08:00–16:30)

$65.00/hr - $75.00/hr

The Home Health RN provides skilled nursing care to patients in their homes, coordinating and delivering patient-centered care in collaboration with an interdisciplinary team. This role focuses on assessment, care planning, medication management, patient education, and accurate clinical documentation.

• Perform comprehensive patient assessments and develop individualized plans of care
• Deliver skilled nursing services, including medication administration and patient education
• Document care accurately using the agency’s charting system
• Coordinate care with physicians and interdisciplinary team members
• Ensure compliance with infection control, safety, and regulatory standards

• Active New Jersey or Compact RN license (in hand at submission)
• COVID-19 vaccination with booster (no exemptions)
• Recent supervisory/manager reference (within 1 year)
• Current Nursys verification required
• Ability to pass EKG and medication exams (if applicable)
• Prior home health or travel experience preferred.
