Job Duties Patient Care

Collects data from patient, family or significant others and records on the Initial Data Base form. Conducts a 24‑hour nursing assessment and provides data to the RN for MDS inclusion.

Contributes to the formulation and updating of the plan of care, and implements the plan of care under the direction of the Registered Nurse.

Provides direct nursing care to assigned patients (eg, medications, treatments, dressing changes, resident care, TPR) according to established standards.

Recognizes changes in patient condition and responds appropriately, notifying the charge nurse as needed.

Documents pertinent information regarding patient responses and nursing interventions in the patient’s medical record.

Administers medication according to hospital policies; monitors age‑specific data including medications, lab/diagnostic results, etc.

Patient Care Related Duties

Applies principles of infection control and safety in the performance of patient care.

Communicates effectively with the charge nurse and other members of the health care team regarding patient needs and changes in condition.

Responds appropriately in crisis situations, capable of prioritization.

Reports significant unit problems to the charge nurse.

Demonstrates understanding and compliance with all regulatory agencies; assumes charge in the absence of a charge nurse.

Delivers patient care responsibilities with knowledge of patient growth and development, appropriate to the ages of the patients served, and complies with the Bill of Rights.

Participates in patient teaching and nursing research activities (CQI).

Provides customer service with respect, flexibility, knowledge, confidence, professionalism, pleasant attitude, patience, and helpfulness; responds in a timely, professional, caring, and respectful manner per Customer Service Performance expectations.

Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Assesses gaps in policies and procedures and creates necessary policies and procedures to fulfill these gaps.

Understands and adheres to the Medical Center’s Code of Conduct.

Orients patients and families to the new environment; develops rapport with patients, family, and significant others.

Orients LPNs, Nurses Aides (Certified Nurse Aides) as required.

Effectively utilizes the hospital computer system per job responsibilities and hospital policy.

Participates on unit committees and in CQI activities as assigned; attends patient care conferences and staff development activities as required.

Assists other personnel as needed.

Performs other related duties as required.

Basic Competencies Education / Licensure

Graduation from an accredited school of practical nursing or equivalent education as described by state education law.

Current New Jersey license:
Licensed Practical Nurse.

Current American Heart Association Basic Life Support certification required.

IV Certification required.

Skills

Successful completion of Progressive Care Orientation Program required.

Clinical nursing skills appropriate for an LPN.

Good oral and written communication skills.

Good interpersonal skills.

Speaks, reads, and writes English to the extent required by the position.

Job Setting / Physical Demands

All patient care areas; contact with patients, visitors, staff.

Frequent prolonged standing/walking.

Lifts, positions, pushes, and/or transfers patients.

Protective clothing worn: face shield, gloves, impermeable gown, particulate respirator.

Base on experience within posted range: $33.22–$39.42 per hour.
