LICENSED PRACTICAL NURSE – FULL‑TIME EVENING LONG TERM CARE (EXTENDED CARE UNIT 11‑7, 21470) – BENGER NEW BRIDGE MEDICAL CENTER

Job Duties
  • Collect data from patients, families or significant others and record on the Initial Data Base form; conduct twenty‑four hour nursing assessment and provide data to the RN for MDS inclusion.
  • Contribute to the formulation and updating of the plan of care and implement the plan under the direction of the Registered Nurse.
  • Give direct nursing care to assigned patients (e.g., medications, treatments, dressing changes, resident care, TPR) according to established standards.
  • Recognize changes in patient condition and respond appropriately, notifying the charge nurse as needed.
  • Document pertinent information regarding patient responses and nursing interventions in the medical record.
  • Administer medication according to hospital policies; monitor age‑specific data including meds, lab/diagnostic results, etc.
  • Apply principles of infection control and safety in patient care.
  • Communicate effectively with charge nurse and other members of the health‑care team regarding patient needs and/or changes in condition.
  • Respond appropriately in crisis situations; prioritize care.
  • Report significant unit problems to the charge nurse.
  • Demonstrate understanding and compliance with all regulatory agencies; assume charge in the absence of a charge nurse.
  • Provide patient‑care responsibilities with knowledge of patient growth and development appropriate to patient ages and comply with the Bill of Rights.
  • Participate in patient teaching and nursing research activities (CQI).
  • Deliver customer service with respect, flexibility, knowledge, confidence, professionalism, pleasant attitude, patience, and helpfulness in a timely professional manner.
  • Maintain established departmental policies and procedures, objectives, quality assurance program, and safety, environmental, and infection control standards.
  • Assess gaps in policies and procedures and create necessary policies and procedures to fill those gaps.
  • Understand and adhere to the Medical Center’s Code of Conduct, Mission, Vision, and Values Statements.
  • Orient patients, families, LPNs, and nursing aides to the new environment; develop rapport.
  • Utilize the hospital computer system as required by job responsibilities.
  • Participate on unit committees and in CQI activities; attend patient‑care conferences and staff development activities.
  • Assist other personnel as needed.
  • Perform other related duties as required.
Qualifications
  • Graduation from an accredited practical nursing school or equivalent education required.
  • Current NJ Licensed Practical Nurse license.
  • Current American Heart Association Basic Life Support certification.
  • IV Certification required.
  • Experience with Med Pass Encore and dressing changes.
  • Successful completion of Progressive Care Orientation Program required.
  • Clinical nursing skills appropriate for an LPN.
  • Good oral and written communication skills.
  • Good interpersonal skills.
  • English proficiency (speech, reading, writing) to the extent required for the position.
Job Setting / Physical Demands
  • All patient‑care areas; contact with patients, visitors, staff.
  • Frequent prolonged standing/walking.
  • Lifting, positioning, pushing and/or transferring patients.
  • Protective clothing: face shield, gloves, impermeable gown, particulate respirator.

Salary commensurate with experience within posted range: $33.22–$39.42 per hour. Differentials: EVEN $1.60, NIGHT $1.70, WEEKEND $1.00.
