LICENSED CLINICAL SOCIAL WORKER- HOME CARE

This position is in a Critical Access, Rural Health Facility comprising 25 inpatient beds with multiple outpatient clinic settings in Paris, IL, and surrounding cities. The role provides social work services under the direction of a physician and in collaboration with the health care team, following agency policies and practices.

$29.91/hr - $47.87/hr

Hours:

24 hours per week, Monday–Friday

• Assess the psychosocial status of patients and/or families to identify factors affecting achievement of goals
• Complete assessments in a timely manner in accordance with agency policies
• Develop written plans to guide information and direction to other health team members
• Document findings, plans, interventions, and patient progress in the clinical record
• Communicate plans and changes to physicians, nursing supervisors, and other agency staff
• Initiate referrals as appropriate
• Prepare social histories, considering cultural factors, financial concerns, and support systems
• Demonstrate teamwork and effective communication to achieve patient, team, and agency goals
• Participate in care conferences and interdisciplinary meetings; share expertise and facilitate referrals
• Educate team members and patients/families about community resources and access
• Provide leadership to the interdisciplinary team to improve services and address unmet patient needs
• Assist in ongoing quality improvement processes
• Perform other related duties as needed; promote the mission and values of the organization
• Ability to work independently and handle difficult situations professionally

• Master’s Degree or Doctorate from an CSWE-accredited School of Social Work
• Licensed in the State of Illinois
• Minimum of one year experience in a health care setting or equivalent
• Home care experience preferred
• Current CPR certification
• Strong written and verbal communication and interpersonal skills
• Knowledge of community resources
• Licensed driver with automobile insurance and reliable transportation

Pay range: $29.917/hour to $47.867/hour (based on experience)
