Rn​/Lpn Clinic- Urogynecology

Horizon Health is a Critical Access, Rural Health Facility located in Paris, IL, with 25 inpatient beds and numerous outpatient clinic settings throughout the region. Since 1968, we have served residents of Edgar County and the surrounding communities with a comprehensive range of health care services.

The Clinical Nurse (RN or LPN) will provide direct patient care in our clinic for scheduled appointments and telephone encounters. The role collaborates with providers to implement individualized care plans and maintains communication with ancillary staff, clinic departments, patients, and families. The Clinical Nurse will also deliver patient education regarding health needs, preventive care, and appropriate self‑management for various age groups.

• Assist with patient flow through the physician’s office.
• Accurately call prescription refills and new prescriptions to the appropriate pharmacy as instructed by the physician.
• Perform triage for patients presenting without an appointment and for occupational health cases.
• Document all telephone interactions concisely, accurately, and completely.
• Provide quality customer service and maintain professionalism with patients, visitors, and staff.
• Apply clinical judgment to assess situations and make sound decisions.
• Advocate for patients, facilitate their entry into the system, and support them in achieving health goals.
• Complete and properly document patient care testing as required by rural health clinic standards and maintain yearly competencies.

• Graduate of an accredited nursing school.
• Current license as RN or LPN;
  Illinois licensure required immediately upon acceptance if from another state.
• Current BLS certification.
• Strong verbal and written communication skills.
• Knowledge of the nursing process and its application.
• Proficiency in basic nursing practices.
• Ability to perform complex tasks in a fast‑paced, high‑stress environment.

Location: 727 E Court St., Paris, IL 61944

Hours: Monday–Thursday 8 am–5 pm;
Friday 8 am–12 pm

RN: $23.855 /hr to $38.168 /hr (based on experience)

LPN: $18.122 /hr to $28.329 /hr (based on experience)
