Position: RN SENIOR CARE - PRN

Horizon Health is a Critical Access, Rural Health Facility with 25 inpatient beds and outpatient clinics in Paris, IL. We have served Edgar County since 1968 through community education, emergency services, and outpatient care. As we expand, our community has grown beyond Paris.

Position Summary

Under the supervision of the Nursing Department, this licensed registered nurse bears responsibility and accountability for the nursing care. The practice includes the nursing process: assessment, planning, implementation, and evaluation. The RN assesses and monitors clients’ health status and physical care needs at admission and throughout the program. The RN performs comprehensive assessments, processes physician orders, reconciles medications, and provides health education for clients and families as needed.

They function as a member of the Multidisciplinary Treatment Team and perform administrative, clerical, and other tasks according to program policy. They demonstrate professionalism in all aspects of performance.

Essential Functions
  • Performs comprehensive admission assessment to obtain presenting complaints, history, and medications.
  • Prepares patient’s Medication Administration Record (MAR) listing allergies and medication responses.
  • Develops a nursing plan of care, implements and documents nursing interventions that are part of the Multidisciplinary Treatment Plan.
  • Consults with Medical Director/Provider, Psychiatrist, Therapists, and other Healthcare providers in the Multidisciplinary Treatment Plan.
  • Attends Treatment Team meetings to discuss and update the Nursing Care Plan section of the Multidisciplinary Treatment Plan.
  • Initiates and conducts individual and group nursing education sessions.
  • Checks for consents and assists patients in understanding their diagnosis and the need for treatment.
  • Communicates relevant clinical and other client care information to the Program Director, Medical Director/Provider, and Treatment Team members as appropriate.
  • Monitors clients to identify health needs, problems, or symptoms; provides prompt, efficient patient care.
  • Coordinates and documents clients’ care in medical or psychiatric emergencies, providing reports to the Emergency Department or other resources.
  • Educates clients regarding bowel activity patterns to maintain regularity and prevent constipation and impaction.
  • Monitors, observes, and records patient behaviors; participates in maintenance of medical records.
  • Ensures a safe environment of care for patients, staff, and visitors.
  • Participates in unit QAPI activities per leadership request; provides patient, family, and caregiver education on health-related topics.
  • Assists clients with personal care needs as requested; coordinates meals and transportation as needed or when unit clerk coverage is unavailable.
  • Completes other duties as assigned by the Program Director.
Position Requirements
  • Graduate of an Accredited College/School of Nursing with an Associate or Bachelor’s degree.
  • Current state RN licensure.
  • Certification in gerontological or psychiatric nursing desirable.
  • Acute-care clinical and/or psychiatric nursing experience required.
  • CPR Certification
  • CPI Certification
  • First Aid Certification
  • Knowledge of community resources
  • Microsoft knowledge/Computer proficiency

Pay ranges from $23.86/hour to $38.17/hour (rate based on applicable years of experience)
