About the Facility

Henry County Health and Rehabilitation is a skilled nursing and rehabilitation facility located in Paris, Tennessee, offering a 136-bed setting with private and semi-private rooms, a life-enrichment program, specialized dementia activities, WiFi, and a fully equipped therapy gym.

Responsibilities
  • Evaluate and treat residents under Medicare, Medicaid, and commercial payer guidelines
  • Conduct comprehensive assessments for swallowing, speech, cognitive-communication, and language deficits
  • Develop and implement individualized treatment plans with functional, achievable goals
  • Document evaluations, daily therapy sessions, progress notes, and discharge summaries accurately and timely
  • Revise treatment plans based on resident performance and progress
  • Educate residents, families, and caregivers on therapeutic exercises, safe swallowing, and carryover strategies
  • Collaborate with nursing staff, interdisciplinary team members, residents, and families to maximize functional outcomes
  • Participate in care conferences, facility meetings, and quality improvement initiatives
  • Maintain knowledge of community resources and assist with discharge planning when needed
  • Engage in continuing education and provide in-service training as required

    Adhere to professional, ethical, regulatory, and facility standards
  • Maintain expected productivity levels and demonstrate strong clinical judgment, communication, and teamwork
Qualifications
  • Master’s Degree in Speech-Language Pathology
  • Certificate of Clinical Competence (CCC-SLP)
  • Active Tennessee SLP license
  • Current BLS / CPR certification
  • Strong observational, verbal, and written communication skills
