Summary: The Case Coordinator coordinates, monitors, and facilitates services designed to reduce suicide risk and improve mental health outcomes. Works closely with mental health professionals and peer support staff to deliver integrated care.

• Serve as the primary contact for clients identified as high‑risk for suicide, conducting comprehensive risk assessments and ensuring each client has an Individual Treatment Plan (ITP) arhrest on suicide prevention.
• Coordinate and lead interdisciplinary team meetings to develop, review, and revise treatment plans in line with client needs.
• Facilitate communication among clients, families, mental health professionals, and peer support specialists for seamless transitions.
• Develop and implement personal crisis safety plans, provide crisis assistance, and ensure clients are connected with emergency services during crises.
• Advocate and assist clients in accessing necessary mental health, physical, and social services including referrals within the Zero Suicide framework.
• Monitor and follow up with clients, tracking progress against treatment plans and offering supportive counseling when needed.
• Accurately document all services in the electronic health record (EHR) and comply with Zero Suicide data collection and reporting requirements.
• Provide education and training on suicide prevention strategies, and participate in community outreach.
• Assist with transportation needs to ensure timely access to care.
• Adhere to federal, state, and organizational policies, participate in quality assurance efforts.

• Clear and accurate communication skills with individuals from diverse backgrounds.
• Team player with a professional, cooperative, and respectful approach.
• Valid driver’s license and clean driving record.
• Basic math skills.
• Successful completion of a Criminal Investigation Background check, DHHR background check, and post‑offer drug screen.

Bachelor’s degree in Human Services or related field with a minimum ofienten credit hours in Human Services disciplines. Prior computer/PC experience with medical records and scheduling systems is preferred. Basic math skills required.

Must be able to talk, hear, sit, use hands and fingers, stand, walk, reach, climb, stoop, kneel, or crouch. Must be able to travel within.
