As a core member of the Stafford Services clinical team, you will be the front-line leader driving optimal outcomes and quality improvement.

• Clinical Autonomy:Provide expert routine and acute medical care directly to SNF residents.
• Comprehensive Management:Conduct thorough assessments, manage complex chronic conditions, and oversee personalized care plans.
• Collaborative Leadership:Partner seamlessly with facility nursing leadership and attending physicians to ensure integrated care.
• Quality Champion:Actively support quality improvement initiatives, focusing specifically on reducing preventable hospital readmissions

with four 10-hour shifts.

• Active and unrestricted
  Nurse Practitioner License
  in the State of Ohio.
• Active
  Nurse Practitioner Certification
  .
• Experience Bonus:Skilled Nursing/Rehab experience is preferred.
• New Grad Welcome:Will consider highly motivated new graduates, though 3+ years of prior RN experience is preferred for this specialized setting.
