Job Summary:

The Home Health Aide shall be employed to provide limited nursing and/or related services in the home, consistent with training, California State Certification, and Federal Code Regulations, Title 20 SSA. The Aide provides personal, environmental, and nutritional services under the orders of a physician and with appropriate supervision by a registered professional nurse.

Essential Responsibilities:

Upholds Kaiser Permanente's Policies and Procedures, Principles of Responsibilities, and applicable state, federal, and local laws.

Assists patients with transfers, ambulation, exercises, and prescribed rehabilitation techniques.

Helps patients with bathroom or bedpan use.

Provides incidental household or environmental services essential to the patient's health care at home, such as doing personal laundry and keeping bedrooms and kitchens reasonably clean (no general housecleaning).

Prepares meals or special diets and may prepare food for the patient's consumption between visits.

Enlists and coordinates assistance of friends or family members and encourages the patient to become increasingly self-reliant when possible.

Reports to the supervisory Registered Nurse any changes in the patient's condition or family situation.

Participates in team conferences.

Maintains records related to job responsibilities.

Performs other duties appropriate to the individual's experience, which, in the opinion of the physician and the Home Health Team, would improve patient care and aide satisfaction.

Assists with the administration of medications that are ordinarily self-administered, specifically those ordered by the physician and delegated by the registered nurse, limited to those medications and patients.

Does not apply topical medications without a physician's order and specific delegation by the RN.

May carry out digital rectal examinations for fecal impactions with an order from MD/RN.

Promotes and improves customer service to internal/external customers, demonstrating skills aligned with the organization's philosophy of providing extraordinary customer relations and quality service.

Assumes other activities and responsibilities as directed from time to time.
