Home Health LVN – Welbe Health

The Welbe Health PACE program helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. Our mission is to serve the most vulnerable seniors with better quality and compassion in a value‑based model. The Home Health LVN plays a vital role in achieving our mission by performing skilled duties at home under CMS approval, bringing convenient and necessary care to our participants’ homes.

• Provide and document in‑person supervision and instruction to home‑care assistants and home health aides assigned to participants’ homes at least as frequently as specified in the Home Health Agency regulations and more often if necessary.
• Instruct the participant, family and caregivers regarding the disease process, self‑care techniques, and prevention strategies, as assigned.
• Conduct and perform home safety assessments as assigned.
• Deliver and document nursing interventions as assigned, accurately responding to physician orders and correctly administering medications and performing ordered tests and treatments.
• Support clinic staff and home‑health staff to manage smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.).
• Be willing to work a varied schedule that may include evenings, nights, weekends and overtime.

• Graduate of an accredited school of nursing.
• Unencumbered LVN license in state of practice.
• Reliable means of transportation.
• Clinical knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.

Base hourly rate $32.61 – $43.04 USD, plus bonus, equity, 401k match, healthcare coverage and other benefits.
