Position: Speech Therapist, Home Health (per diem)

We're looking for a per diem Speech Therapist to support our Covina Home Health branch to pick up about 3-5 visits per week on the west end of our market (Pasadena to Covina).

We pay the following per/visit rates:

  • $146 per/Start of Care (including mileage reimbursement and Oasis bonus)
  • $101 per/Evaluation (including mileage reimbursement)
  • $91 per/Routine(including mileage reimbursement)

The impact you will make:
  • Complete initial and on-going assessments and evaluation to identify the client’s level of functioning and ability to communicate, including OASIS assessment at appropriate time points.
  • Combine knowledge of speech, hearing and language with planning and implementing programs for the management of communication disorders.
  • Develop the plan of care in consultation with the physician and other care team members.
  • Instruct other health team personnel, including Home Health Aides and the patient s family, in Speech Therapy Program.
  • Participate in in-service education, utilization review and team conferences.
  • Provide discharge planning, as appropriate.
  • Evaluate effectiveness and outcomes of care / response to therapy interventions.

Who you are:
  • Master’s or doctoral degree in Speech-Language Pathology and meets qualifications commensurate with § 484.115(n) (Condition of participation: Personnel qualifications / Speech-Language Pathologist).
  • Current, active license as a Speech Pathologist by the California Board of Medical Quality Assurance.
  • Core competency per skills checklist.
  • At least one year of experience in speech therapy.
