Responsibilities:- Skilled in Trach and vent
- Provide direct patient care including wound care, medication administration, and vital sign monitoring
- Assist in developing and implementing individualized patient care plans
- Collaborate with RNs, physicians, and interdisciplinary healthcare teams to ensure optimal patient outcomes
- Document patient assessments, treatments, and progress accurately in medical records
- Educate patients and families on health management, medications, and post-care instructions
- Monitor and report changes in patient condition to supervising nursing staff or physicians
- Maintain a safe, clean, and compliant care environment in accordance with healthcare regulations
- Active and valid LPN license in the state of Maryland
- Graduate of an accredited Licensed Practical Nursing program
- Previous clinical experience in a healthcare, home health, or long-term care setting preferred
- Strong knowledge of nursing procedures, medical terminology, and patient care standards
- Current CPR/BLS certification required
- Excellent communication, organizational, and critical thinking skills
- Compassionate, patient-centered approach with a commitment to quality care
