LPN Med Care Manager

The Licensed Practical Nurse (LPN) Medication Nurse provides licensed nursing support in a senior living environment, focusing on safe medication administration, health monitoring, and precise clinical documentation.

• Administer medications and treatments according to physician orders, adhering to the Six Rights of Medication Administration.
• Accurately document in MAR/TAR in real-time and transcribe physician orders to support care plan updates.
• Monitor residents for symptoms, administer medications for symptom relief, and report any refusals or adverse reactions promptly.
• Conduct Service & Health Updates (SHUs), observe physical, emotional, or cognitive changes, and report findings appropriately.
• Maintain medication carts, complete narcotic counts, and ensure compliance with regulatory standards.

• Active LPN license in good standing, CPR, and First Aid certifications.
• Strong assessment and documentation skills.
• Experience in senior care or medication administration preferred.
