Summary

ESD 123 is looking to hire two qualified individuals to serve as the Student Support Care Coordinators serving Benton and Franklin Counties. This position provides community‑based care coordination and case management services to youth and families experiencing complex social, behavioral health, educational, and basic needs barriers. This role focuses on engaging youth who are at risk of school disengagement, justice involvement, housing instability, or unmet health‑related social needs, and supporting their successful connection to education, behavioral health services, healthcare, and community‑based resources.

This position is funded through the Greater Health Now (GHN) Community Care Hub from 1/1/2026–12/31/2026, with potential for ongoing funding beyond this period.

The Student Support Care Coordinator functions in alignment with GHN’s Community‑Based Care Coordination Standards (Engage, Assess, Support, Connect) and works collaboratively across education, healthcare, juvenile justice, and community systems to ensure youth and families receive timely, coordinated, and equitable support.

Essential Functions
  • Engages youth and families through outreach, referral follow‑up, and relationship‑building to support participation in community‑based care coordination services.
  • Conducts comprehensive assessments, including Health‑Related Social Needs (HRSN) and psychosocial screenings, to identify educational, behavioral health, housing, transportation, food access, and other basic needs barriers.
  • Assists in developing individualized care plans and education re‑engagement plans for youth who are suspended, expelled, justice‑involved, or at risk of disengaging from school.
  • Coordinates warm hand‑offs and referrals to behavioral health providers, healthcare services, recovery supports, housing resources, food assistance, transportation options, and other community‑based services.
  • Monitors youth progress toward care plan and educational goals through regular follow‑up, documentation, and collaboration with service providers, schools, and families.
  • Meets with students, families, school counselors, administrators, healthcare partners, juvenile justice staff, and community providers to facilitate coordinated service delivery and reduce barriers to access.
  • Serves as a liaison between school districts, community agencies, and juvenile detention or justice systems to support re‑entry planning, continuity of care, and educational transitions.
  • Maintains active communication with school district staff and community partners across Benton, Franklin, and surrounding service areas; advocates for youth and families within education, health, and social service systems.
  • Participates in multidisciplinary meetings, advisory groups, care coordination reviews, and GHN‑required trainings to support integrated service delivery and fidelity to care coordination standards.
  • Attends re‑entry planning meetings, case staffing sessions, and visits community‑based and detention settings as necessary to assess youth needs and coordinate services.
  • Completes required documentation, progress notes, care plans, assessments, and reports in GHN’s Client Management System (CMS), ESD 123 systems, and other required reporting platforms while maintaining confidentiality and compliance with FERPA, HIPAA, and applicable state and federal regulations.
  • Maintains knowledge of available community, educational, behavioral health, and healthcare resources; provides information and navigation support to youth, families, and partners.
  • Conducts screenings and ongoing reassessments to identify emerging needs and adjust care plans accordingly.
  • Maintain confidentiality and follow all applicable board policies and procedures.
  • Performs related duties consistent with the job description and assignment.
