SUMMARY

The CDC 123 seeks to hire two individuals to serve as Candy Mountain Academy (CMA) Case Managers. The role is part of the student intervention team and works directly with students who face significant social, emotional, and behavioral challenges. Responsibilities include academic advising, assessments, career guidance, intervention, advocacy, and referrals to social and health services for youth and young adults with emotional‑behavioral disabilities.

The position supports students’ transition to less restrictive educational settings and the community.

ESSENTIAL FUNCTIONS
  • Establish and maintain collaborative relationships with community resources and coordinate services and referrals.
  • Support the goals in each student’s IEP, Behavior Intervention Plan, or Student Success Plan.
  • Provide academic advising, career guidance, employment assistance, and referrals to service agencies.
  • Meet regularly with students to foster growth, motivation, and skill development.
  • Support students in setting personal goals, monitoring progress, adjusting goals, and planning for the future.
  • Collaborate with youth, families, guardians, and positive adults to establish and support student goals.
  • Collaborate with community agencies and advocate for students to increase access to resources.
  • Maintain monthly status check documentation and required records.
  • Maintain cooperative relationships with education, social, legal, and health service agencies.
  • Assist in planning, organizing, coordinating, and developing programs and events.
  • Attend partnership meetings to raise awareness of programs and strengthen community connections.
  • Provide program support for staffing, training, documentation, and resource distribution.
  • Assist the administration team with student support services, behavior programming, data collection, assessment coordination, transition support, and scheduling.
  • Respond to student crisis situations following CMA procedures to maintain safety protocols.
  • Manage program resources and data for effective utilization and planning.
  • Support new staff with training, data analysis, and program implementation.
  • Facilitate direct services offered by the program in alignment with goals and student needs.
  • Respond to inquiries to solve problems and provide information.
  • Maintain confidentiality and follow all board policies and procedures.
QUALIFICATIONS
  • Bachelor’s degree in social work, education, counseling, or related field preferred.
  • At least one year of experience providing case management, counseling, or direct services to students with significant behavioral challenges.
  • Strong interpersonal skills and conflict management techniques (e.g., restorative practices).
  • Computer skills: MS Word, Excel, Outlook, and internet.
  • Experience working effectively with diverse populations.
  • Experience providing referral, advocacy, and case‑management services.
  • Valid WA State Driver’s license, reliable personal transportation, and proof of insurance.
  • Demonstrated ability to utilize time‑management techniques.
  • Flexibility and adaptability to programming changes.
  • Ability to work as an ESD team member supporting the ESD Strategic Plan and agency initiatives.
  • WSP/FBI background clearance required.
