Overview
The Support Services Counselor II (SSVF) will provide case management services to veterans and families in the OIC of Washington grant‑funded program. The role focuses on developing individualized housing plans, coordinating resources, and ensuring veterans achieve permanent housing stability.
Essential Duties- Perform primary case management responsibilities for program clients, community stakeholders, and other agencies.
- Coordinate, prepare, and maintain required charting and documentation in HMIS/Case worthy on a timely basis.
- Conduct comprehensive assessments, develop Individualized Housing Stability Plans, and monitor progress at 30, 60, and 90‑day milestones.
- Identify affordable housing units, coordinate move‑in logistics, and facilitate lease and utility set‑up.
- Maintain accurate case notes and facilitate referrals to community linkages, VA benefits, mental health, legal advocacy, and financial resources.
- Collaborate with OIC SSVF team to reach enrollment and housing stabilization goals, participate in daily debriefs, supervisory meetings, and trainings.
- Market the program to landlords, mediate landlord‑client issues, and conduct outreach activities (resource events, street outreach, community connections).
- Perform routine backups, operate telephone system, operate general office equipment, and manage mail and messages.
- Assist with conflict resolution, client needs, and continuation of support after stabilization to ensure 90‑day housing maintenance.
- A.A. or B.A. degree in Social Services or related field.
- 1–2 years of case management experience, preferably in veteran services.
- Strong communication skills, both written and verbal.
- Proficient in computer applications (word processing, spreadsheets, scheduling tools).
- Ability to work independently, manage multiple projects, and maintain confidentiality.
- Occasional standing, walking, and sitting; frequent talking and hearing.
- Occasional vehicle operation under varying weather conditions.
- Occasional lifting up to 10 lbs; occasional reaching, pushing/pulling, stooping.
- Pass a criminal background check and pre‑employment drug test.
- Maintain a current driver’s license and reliable transportation.
- Employment contingent on executive approval.
Salary: $23.85–$31.12 per hour.
