- Lead the facility's social services program and advocate for resident well-being
- Complete psychosocial assessments and support individualized care planning
- Assist residents and families with adjustment, discharge planning, and transitions of care
- Connect residents and families with community resources and support services
- Collaborate with interdisciplinary team members to promote positive resident outcomes
- Help ensure compliance with social services regulations and documentation requirements
- Bachelor's degree in Social Work preferred
- Two years of social work experience required
- Healthcare or skilled nursing experience preferred
- Knowledge of state and federal regulations
- Strong communication, assessment, and documentation skills
- Basic computer proficiency and ability to pass employment screenings
