Job SummaryThe Case Manager functions as an integral part of the treatment team and provides primary social service care to a caseload of residential addiction treatment clients in residence.
Responsibilities- Provide case management for a caseload of residential addiction treatment female clients with children in residence, including the following tasks:
- Serve as a liaison between Straight & Narrow, Drug Court/Probation and all other social service agencies, including the scheduling of Case Conferences as necessary.
- Ascertain whether client needs birth certificates, social security cards or an identification card. Determine status of their TANF, food stamps, Medicaid benefits (if applicable).
- Send for ad/or make appointments for client to go for any of the above that are missing.
- Open client’s welfare case in Passaic County. Obtain any documents necessary to do this. Close client’s welfare case in sending county (if applicable).
- Obtain prior authorization for clients receiving Medically Assisted Treatment (MAT)
- Open client’s Medicaid case immediately upon receipt of all required documents (if applicable).
- Run Medicaid reports on all active clients monthly and renew client’s Medicaid if applicable
- Make contact with client’s relatives to bring items needed and/or go to the store and purchase these items.
- Go over client’s shopping lists monthly. Remove any inappropriate items. Check client’s current supplies to see that they are not over-ordering. When client returns from store, compare what is on the list with what he has bought.
- Educate/Assist client with regards to money management and budgeting strategies.
- Schedule weekly meetings with clients and provide them with phone calls to their probation officers, etc.
- Attend any Case Conferences that are scheduled.
- Schedule outside appointments for welfare, WIC, social security, Probation visits, Section 8 applications, job search, trips to IOP for interviews, etc.
- Escort clients on shopping trips, to appearances, on interviews related to Section 8 housing, job interviews and at all times when they are looking for an apartment.
- Apply for Section 8 housing for the client, obtaining all necessary documents in order to complete her application.
- When the client secures an apartment, apply for Emergency Assistance. Make arrangements to have client’s lights, gas and phone turned on. Make sure deposits are made for client’s rent and utilities.
- When Section 8 housing is not available, work with referral source to find alternative housing.
- Arrange an appointment for client to be interviewed and complete necessary paperwork at IOP before client is discharged. Coordinate with IOP to arrange for a medical appointment so that there is no gap in medical services if client is taking medication. Identify a child care provider in a timely manner and make application for the client’s child/children.
- Close out client’s account at S & N before he leaves, transferring all personal documents to him. Fill out form that gives client a checklist of what to do upon discharge and give her a letter to take to her first welfare appointment.
- Close out client’s Passaic County welfare case (if applicable).
- Facilitate case management group and didactics as scheduled by supervisor
