Licensed Practical Nurse (LPN)

Alvita Care is seeking compassionate, reliable, and professional Licensed Practical Nurses (LPNs) for per diem floor staffing opportunities in Patchogue, NY. • Most available shifts are 3 PM–11 PM, on weekdays and weekends.
• Per diem positions with flexible hours. Full-time or part-time hours may be available, based on facility staffing needs.

• Pay Rate: $35–$40/hour, based on experience and assignment

• Provide quality nursing care and support to residents.
• Administer medications and treatments within the scope of the LPN license.
• Monitor residents' conditions and report changes to the appropriate healthcare professionals.
• Maintain accurate and timely documentation using the facility's charting system.
• Follow facility policies, procedures, and infection control protocols.
• Collaborate with other healthcare professionals to promote resident safety and well-being.
• Assist with other nursing duties as assigned within the scope of practice.

• Valid New York State LPN license required.
• Previous nursing experience preferred.
• Long-term care or facility experience preferred.
• Strong communication and documentation skills.
• Authorization to work in the United States.
• Updated medical and vaccination records, or willingness to obtain them.
• Completion of required drug screening and health documentation.
• Successful completion of Alvita Care's onboarding process before starting an assignment.
• Ability to meet facility scheduling and staffing requirements.
