Job Description
The Patient Navigator is responsible for identifying patients who require assistance with coordination of care. Works collaboratively with clinical staff of the ambulatory departments to facilitate timely transitions into care. Assists patients with scheduling of appointments while also identifying barriers to care. Improves access gaps which involves developing knowledge of the outpatient care setting and community services. Is accountable for successful tracking and linkage to care.
Qualifications
Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Bachelor's degree in a Social Service field or equivalent experience in the field of HIV. One to two years of work related experience and three to six month on the job training and orientation.
