Residential Case Worker PD – Brown University Health

- Available to cover shifts as assigned at any residential program, including travel between programs.
- Facilitate ADL skill‑building for residential living and for community integration through supervising and teaching clients daily medication routines, budgeting, self‑preservation skills, transportation, etc.
- Provide opportunities for skills practice and collaborative learning.
- Participate on a multidisciplinary team in the development and implementation of client treatment plans and progress reviews, engaging clients in the planning and evaluation process.
- Provide, coordinate and monitor treatment services for all assigned clients, including contacts with the client, family, other Gateway staff, consultants, and treatment plan designated staff.
- Monitor the effectiveness of care provided by the program or through other service providers, comparing service provided with service outcomes.
- Provide shift coverage including household maintenance, cleaning and meal preparation, crisis resolution, conflict resolution and symptom management.
- Attend supervision and team meetings as required.
- Maintain current knowledge and updates of agency policies and procedures.
- Ensure Gateway office areas, residential programs are safe and secure via checking door and window locks and knowing the whereabouts of all residents at all times during the shift.
- Schedule treatment plan meetings and facilitate them, utilizing assessment information, diagnostic results, medical records, client and family participation, and other human service providers to develop and document assessments and treatment plans/reviews in accordance with agency policy and procedure.
- Screen for substance abuse and provide assessment with indicated treatment planning, counseling/education and referral.
- Perform other duties as assigned or needed and remain in shift until appropriately relieved by another staff member.
- High school diploma or GED required.
- Maintain flexibility toward work schedules; availability for first and second shift, holidays and weekends.
- Ability to perform routine household chores involving lifting and bending.
- Ability to communicate with clients and staff face to face and by telephone.
- CPR certification and first aid training; monitor client’s vital signs as required.
- Valid driver’s license and capacity to use program van or personally owned vehicle to transport clients to day‑activity programs, medical appointments, other appointments and clinical appointments when necessary and to respond to medical emergencies and disaster evacuations.
- Ability to read, comprehend and follow instructions, correspondence and memos.
- Proficiency in completing all clinical and administrative documentation.
- Strong oral and written communication skills; ability to present information in one‑on‑one and group situations.
- Ability to access emergency resources and manage crisis situations.
- Compliance with agency ethics, policies and procedures.
- Physical ability to lift up to 35 pounds, walk, stand, stoop, kneel, crawl, and handle hands‑to‑finger tasks; vision requirements include close vision, distance vision, peripheral vision, depth perception.
Pay Range: $17.00 – $27.13 per hour
