CASE MANAGER 3 - EARLY INTERVENTION UNIT

Department of Economic Security (DES), Division of Developmental Disabilities (DDD), seeks an experienced and highly motivated individual to join our team as a Case Manager 3 in an Early Intervention Unit. The role supports families with developmental disabilities by building relationships, coordinating care, and facilitating interdisciplinary planning to help children meet their developmental goals.
Essential Duties and Responsibilities- Drive vehicle to conduct home visits, meeting with individuals and families to assess dynamics, priorities, resources, and concerns, and to make appropriate referrals to community agencies.
- Convene and facilitate multi‑disciplinary Individualized Family Service Plan (IFSP) meetings (minimum every 6 months for each child) to support families in achieving developmental outcomes within the child's natural environment.
- Offer families (at the IFSP) AzEIP surveys; educate parents about rights, responsibilities, and procedural safeguards; teach advocacy skills; and assist with grief processes when applicable. Collaborate directly with the Department Of Child Safety (DCS) to meet child needs while in the DCS system. Assess each child's progress regularly.
- Methods, procedures, and techniques used in the development of Individualized Family Service Plans.
- Health Insurance Portability and Accountability Act (HIPAA) compliance.
- Federal and State statutes and agency policies related to the care of individuals with developmental disabilities.
- Principles and practices of case management.
- Initiating, developing, and maintaining member service plans.
- Oral and written communication.
- Conflict resolution.
- Work independently and within diverse work teams.
- Lead a team of Support Coordinators in the absence of the supervisor.
- Organize and manage caseload.
- Drive a motor vehicle.
- One year of experience working with people with Developmental Disabilities preferred.
- A minimum of a bachelor’s degree in Early Childhood, Early Child Special Education, Family Studies, or a closely related field, with course review and approval by ADES/AzEIP. Degree options include Psychology, Social Work, Elementary Education, or Sociology.
- Valid driver’s license and completion of required training for driving duties.
- Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41‑1968.
- Background and reference checks, including registries from the Department of Child Safety and Adult Protective Services.
- Successful completion of the Electronic Employment Eligibility Verification Program (E‑Verify).
