• Provide individualized care and support to clients in their homes, including assistance with daily living activities, medication management, and personal care
• Develop and maintain strong relationships with clients, families, and caregivers to ensure seamless communication and care coordination
• Collaborate with care teams to develop and implement comprehensive care plans that meet each client's unique needs and preferences
• Ensure a safe and healthy environment for clients by maintaining a clean and organized living space, managing medication, and addressing any concerns or issues that arise
• Document client records and reports, ensuring accurate and timely communication with care teams and families
• Participate in ongoing training and education to stay up-to-date on best practices and industry developments

• High School Diploma or equivalent required
• 1 year of experience in healthcare or caregiving preferred
• Ability to lift 25 pounds
  , stand for long periods, and perform tasks that require bending, stooping, and reaching
• Valid driver's license and reliable transportation (if applicable)
• Excellent communication and interpersonal skills
